Vaccine and medical supply governance

In a health emergency, vaccines, diagnostics, therapeutics and protective equipment must be developed, approved, made and delivered at speed. Manufacturing is concentrated in a few countries, and allocation becomes a political contest. This page sets out how medical countermeasure governance works and where it fails, the records that show access and supply, and who decides.

01In brief

The short answer.

Vaccine and medical supply governance is the set of rules and institutions that decide how vaccines, diagnostics, therapeutics and protective equipment are approved, made, bought and allocated in emergencies. Concentrated manufacturing, advance purchases and regulatory capacity shape access. Regulators, health ministries and pooled mechanisms hold the decisions.

03The question

Vaccine and medical supply governance: how the risk develops.

Access, procurement authority and resilience. The dossier below sets out how the risk develops, what is measured, where the data come from, how it is modelled and who decides.

Mechanism

Manufacturing is concentrated

A few countries and firms produce most vaccines and key inputs. Export restrictions and advance purchases by wealthy states leave others waiting.

Mechanism

Regulatory capacity sets pace

Countries without mature regulators rely on WHO prequalification or reliance pathways. Duplicated reviews and unclear emergency use rules delay access.

Mechanism

Advance purchase decides allocation

Contracts signed before products exist reserve supply for those who can pay and bear risk. Pooled mechanisms depend on donors and on manufacturers honouring commitments.

Mechanism

Last mile limits delivery

Cold chain, workforce and demand decide whether doses become vaccinations. Supply without delivery capacity leads to expiry and waste.

Terms used on this page.

Each term as this page uses it. Where a standard-setting body defines the term, the wording follows that body.

TermDefinition
WHO prequalificationWHO's assessment of health products against quality, safety and efficacy standards, used to guide UN and national procurement.
Regulatory relianceA regulator taking account of a trusted regulator's assessment when reaching its own decision.
Advance purchase agreementA contract to buy a product before it is approved, reserving supply in exchange for funding or risk-sharing.
05Indicators

What is measured, and how well.

Each indicator carries its unit, its cadence, the kind of source behind it and the evidence label a value would carry. No values are shown: a programme supplies them under its own authority.

09Core questions

What experts ask.

The questions concern access, authority and resilience.

  • Who has authority to purchase and allocate countermeasures, nationally and through regional or international mechanisms?
  • How are allocation priorities set, and are they transparent and fair?
  • How do export restrictions and advance purchase agreements affect access for others?
  • How is regulatory authorization coordinated across countries in emergencies?
  • Where is manufacturing capacity located, and how can it be diversified?
10Mechanisms

Mechanisms and drivers.

Manufacturing capacity for vaccines and many medicines is concentrated in a limited number of countries and firms. In emergencies, countries with purchasing power and domestic production can secure supply first, while others wait. Pooled procurement and regional manufacturing are among the responses to this inequity.

Regulatory processes determine how quickly products reach people. Reliance on assessments by trusted regulators and emergency procedures can speed access but require trust and capacity. Allocation frameworks must balance health need, risk of exposure, protection of health workers and equity.

13Questions

Questions and answers.

What is WHO prequalification?
A WHO assessment of medicines, vaccines and diagnostics against quality, safety and efficacy standards, used by UN agencies and many countries to guide procurement.
What is regulatory reliance?
When a regulator takes account of another trusted regulator's assessment in reaching its own decision, saving time without giving up its authority.
Why did vaccine access differ between countries?
Because of advance purchase agreements, concentrated manufacturing, export restrictions, regulatory capacity and delivery systems, each documented in reviews of the COVID-19 response.
Why were countermeasures distributed unequally in past emergencies?
Mainly because manufacturing was concentrated and countries with purchasing power secured supply in advance, while allocation mechanisms for others were under-resourced.
What is pooled procurement?
An arrangement in which several countries or purchasers buy together, which can improve bargaining power, reduce prices and widen access for smaller buyers. It requires agreement on governance, financing, product specifications and how supplies are allocated among participants when demand exceeds supply.
Does the platform procure or allocate medical supplies?
No. GRF does not finance or procure. The platform convenes institutions and publishes governance analysis.
